I think it might be starvation.

Neither of those were options we considered her three threats to be initially.

From her memories I do know starvation is an issue for her, hence my worry that this is what's going to be the most distressful for her. The whale earlier could also be a hint toward eating, rather than a tootache or drowning.
But this is entirely based off this being an image of food. Do we want to ask our yes or no questions for this ...?
